Does anyone have any information on this incident?

MR. CAREY...
I can’t remember dates or location, an event happened aboard ship.  A fuel line was cut loose too soon and started to whip around.  It hit a crewmember and he went overboard.  I was the one behind the line.  Trying to find anyone who might have some information on this matter.  Just happened to locate your name.
